Just because an insect takes up residence in a defect does not mean that it caused the defect in the first place.

Sounds like it could be "kanzan dieback", the cause of which is not fully understood. It often follows particularly cold winters and may be associated with the bacterium Pseudomonas syringae. Not a lot that can be done - just avoid kanzan in the future.
