Thinning Japanese Cherry tree

Last year our Japanese Cherry Tree's leaves were much smaller than usual and the same thing has happened this year. It seems that only around half of the leaves have even come out and the ones that have are about half their normal size. This tree is around 10 years old and we have never really pruned it except for a few branches here and there. The trunk seems to be very large and then where the tree starts to sprout its limbs it seems to be smaller than what the trunk is.
