Included Bark

Is "included bark" the same as "fungus infested"?
Would you be able to observe an "included bark" limb from the ground with a limb that is in full foilage?
What qualification would someone need to identify a tree limb with "included bark" such as an arborist, tree trimmer, etc..
Your immediate and timely response would be greatly appreciated.
