These sort of structures are common on older Thuyas David and I have
never seen any detachments.

On a recent survey I was looking at a thuja with a stem dbh of 90cm - a
lateral branch at 2.5m height had a diameter of 76cm and was 9m in
length; I can't find any guidance notes, can anyone offer sound advice
whether this may be a structural worry?

No evidence of splitting or stress fractures at present.
