The point of a risk management system is to manage risk, not to
eliminate it. There will always be accidents. There will always be
accidents involving trees. If an accident happens, it does not mean your
system failed. It could just mean that you, and the victim, were very
unlucky.

As I pointed out in a recent letter to the AA Newsletter, if you buy a
lottery ticket you have a very low (1 in 14 million) chance of winning
the jackpot on Saturday evening. If you do win it does not mean that
your chance of winning was any higher prior to the numbers coming up.

Likewise, if an accident occurs it does not mean that the risk of the
accident occurring prior to the event was any greater. Hindsight is a
wonderful thing. But not actually much use in the real world. Some
accidents are preventable. Some are not. Other professions recognise
this - my wife, who's a midwife, recently attending a training session
entitled "Meconium happens" (look it up if you don't get it) - it's
about time we recognised it too.
